The Canadian Red Cross Society, a non-profit, humanitarian organization dedicated to helping Canadians, as well as the most vulnerable throughout the world, is seeking Health TAPESTRY Community Connector Volunteers for the Community Health and Wellness Tapestry Program.  Reporting to the Volunteer Coordinator, the Community Connector Volunteer will work to help people stay healthier for longer in the places where they live. The Community Connector Volunteer will work in this adaptable model of health care to connect senior clients with their primary health care team and their community, through the support of technology.
Key Responsibilities
Visits clients in their homes to conduct wellness assessment surveys/questionnaires
Assists clients in personal health-related goal identification
Provides social connection to older adults
Communicates with volunteer organizations, family health team, research team, and/or community based organizations
Utilizes a tablet computer and other technology to provide information to identified organizations
Maintains up to date knowledge of health programs and services and acts as resource in the community through the use of approved information and referral sources
Works closely with program coordinator to schedule client appointments
Completes feedback and reports; and submits information to the volunteer coordinator
The incumbent must have the commitment to the Fundamental Principles of the Red Cross Movement
A good understanding of services in the community is considered an asset
An interest in building partnerships based on community identified needs
Knowledge about and involved in their community or a desire to become involved or learn more
Ability to communicate in English required
Active listening skills
Ability to work independently with minimal supervision
Ability to work within a team
Ability to set clear boundaries
Friendly interpersonal skills
Adherence to privacy and confidentiality guidelines
Good organizational skills
Empathy and compassion with the capacity to remain calm in unexpected situations
Enjoyment of socializing with older adults
Experience working with a diverse population an asset
Computer literacy
Standard First Aid Certification required (or willingness to take training)
Working Conditions
Visit clients at their home (average visit time from 1 to 2 hours) - the conditions of the client’s home may not be known prior to the visit occurring
All visits will be within the assigned community
Local travel is required. Must have reliable transportation by personal or public transit
Meetings and supervision may be in person, by telephone or through other electronic means
Must have access to a telephone, email and internet
Time spent engaging in activities will vary by a volunteer’s interest and availability with the average person committing 6 hours per month
Required to take on-line and face to face training
Will be required to take tuberculosis test (reimbursements available)
